Follow these three simple steps to prepare for your first appointment.

    Choose a family physician near you

    See which ONcare doctors are accepting new patients at the clinics closest to you, and pick the one that suits you.

    Fill in the intake form

    Complete our secure intake form with your medical history and basic information, so your doctor can prepare for your visit.

    Book your Meet & Greet

    A brief first visit (about 15 minutes) with your new doctor. Bring your OHIP card or IFHP coverage document.

Essential information for a smooth clinic experience.

Zero Tolerance Policy

We maintain a safe, respectful environment. Aggressive, abusive or disruptive behaviour — including verbal threats, harassment or inappropriate language — may result in cancellation of your appointment and dismissal from the clinic.

No-Show Policy

Missed appointments affect access for other patients. A $50 CAD fee applies to your next visit if you miss an appointment without at least 24 hours’ notice.

Prescription Policy

Our clinic does not prescribe narcotics or controlled substances. We focus on evidence-based, responsible prescribing to keep patients safe.
